Protection Dog Training and Protection Sport Liability Agreement

Acknowledgment and Assumption of Risks:

​

I, the undersigned participant, understand and acknowledge that there are inherent risks associated with my participation, and my dog's participation, in protection dog training and protection sport activities provided by Dog Tribe, DBA Chicago Pet Services, Inc. These risks include, but are not limited to, physical injury to myself, my dog, or others, property damage, and other potential hazards that may arise during training or sport activities.

Release of Liability:

In consideration of being permitted to participate in these activities, I hereby release and forever discharge Dog Tribe, DBA Chicago Pet Services, Inc., its owners, employees, agents, contractors, and successors from any and all liability, claims, demands, actions, or rights of action, damages, and attorneys' fees that I, my heirs, or my legal representatives may have now or in the future. This release encompasses any and all claims arising from or connected with my participation, my dog's participation, or my presence on the premises of Dog Tribe, DBA Chicago Pet Services, Inc., excluding those claims arising from the gross negligence of Dog Tribe, DBA Chicago Pet Services, Inc.

In the event that Dog Tribe, DBA Chicago Pet Services, Inc. is adjudged by a court of law to be grossly negligent, Dog Tribe, DBA Chicago Pet Services, Inc. shall be liable only for actual damages and shall not be liable for punitive, special, or consequential damages, nor for damages for pain and suffering, infliction of emotional distress, or similar damage claims.

Dog's Information:

I agree to disclose any allergies, special dietary needs, behavioral concerns, or medications my dog may require during activities at Dog Tribe, DBA Chicago Pet Services, Inc. I understand that it is my responsibility to provide accurate and up-to-date information regarding my dog's health and behavior.

 

Emergency Veterinary Care:

In the event that my dog becomes ill or injured while participating in activities at Dog Tribe, DBA Chicago Pet Services, Inc., reasonable efforts will be made to contact me using the provided contact information. However, if Dog Tribe, DBA Chicago Pet Services, Inc. is unable to reach me, I authorize them to seek appropriate veterinary care for my dog. I accept full responsibility for any associated expenses, and I acknowledge that Dog Tribe, DBA Chicago Pet Services, Inc. will not cover any portion of veterinary expenses.

Payment and Billing:

I agree to pay for my dog's participation in protection dog training and protection sport activities at Dog Tribe, DBA Chicago Pet Services, Inc., as well as any additional services requested, such as grooming, drop-off, and pick-up services. I understand that a valid credit card must be kept on file with Dog Tribe, DBA Chicago Pet Services, Inc. at all times for payment in the event of medical emergencies or other charges.

 

Use of Visual Images:

I grant Dog Tribe, DBA Chicago Pet Services, Inc. permission to use my dog's visual images and statements in promotional materials, including but not limited to newsletters, posters, and other marketing materials, without compensation or further permission.

Electronic Signature Disclaimer:

I understand and acknowledge that by checking the box below and submitting this document electronically, I am providing my electronic signature, which has the same legal effect as a handwritten signature. I consent to the use of electronic signatures and accept the terms and conditions outlined in this agreement. I confirm that I have read, understood, and agree to be legally bound by the content of this document.
